一、假條的上方應(yīng)該按照正式的格式寫上如下信息：

From：請(qǐng)假人 From: Lynn Chen, Financial Department Date：寫假條的'日期(注意不是請(qǐng)假的日期) Date: April 2nd, 2006
Subject：寫上請(qǐng)假字樣 Subject: Casual Leave of Absence
二、在您的請(qǐng)假信第一段，應(yīng)該開門見山但是有禮貌地提出請(qǐng)假。第一段要中心明確，寫清您要請(qǐng)假的日期。
